At Endurance Therapeutics, your privacy and the protection of your personal information is a top priority. We are committed to collecting, using, and disclosing your information responsibly, in compliance with Ontario’s Personal Health Information Protection Act (PHIPA) and the Personal Information Protection and Electronic Documents Act (PIPEDA).

Payment information, including extended health benefits and insurer details
Appointment history and health goals

To remind you of upcoming appointments or changes to your care

3. Protecting Your Information
We take safeguarding your information seriously. We have implemented physical, administrative, and technical measures to protect your data, including:
Secured storage of paper records and locked office spaces
Password-protected and encrypted electronic records
Staff training on privacy and confidentiality policies
Strict access controls and confidentiality agreements with third-party service providers
Secure disposal of outdated personal information
Please note: Email is not a secure method of communication. We do not send personal health information via email without your consent.
4. Retention & Destruction
We retain your health records for a minimum of 10 years after your last visit (or 10 years past your 18th birthday if you were a minor at the time of care) as required by law and regulatory bodies. After this period, records will be securely destroyed.
5. Access to Your Information
You have the right to access and review your personal health information. Upon request, we will provide you with access to your records within a reasonable time. If you believe any information is inaccurate, you may request a correction.

This policy may be updated from time to time to reflect changes in regulations or our clinic practices. Any updates will be posted on our website and available in the clinic.
